We are looking forward to seeing you here. In order to help us get ready for your visit,
please review and complete the New Patient Form prior to your first visit here. The
form can be found in this website. Simply click on "Patient Resources", then go to
"New Patient Information" and look under "New Patient Form". Also for your visit,
please bring your insurance card/s with you so we can make a copy of it, as well as
your referral authorization from your primary care physician, if it is required by your
insurance. Please note that if your insurance requires a referral authorization form and
you don't have it with you, we will have to reschedule your appointment, unless you
pay at the time of the visit. In that case, you will get reimbursed directly from your
insurance, whatever they will be willing to pay without the referral.
